Job Description:
Maintain the policy and procedures established for the clinical record keeping of the facility. This includes release of information, supervision of unit clerks and medical secretary staff, and reports and records as required by law and accrediting agencies. Maintain master Patient Index for current and previous residents. Obtain charts on discharged residents from nurses stations, close in standard manner, obtain all signatures, and file in record storage room when complete. Initiate PAE Form and track throughout the various stages. Follow-up when further action needed for level of care change, end-date, or update needed. Code final diagnosis and maintain disease index. Maintain report for assisting with Joint Annual Report. Handle all requests for medical information release, answer subpoena and respond according to policy. Participate in required meetings, including but not limited to, Performance Improvement, Department Head/ Operations Team, and others as requested. Maintain all medical records to meet all Medicare, Medicaid, state licensing, and JCAHO requirements. Conduct and participate in in-service training and staff orientation. Responsible for staffing the department and supervision of Assistant Director of Medical Records, Medical Secretary, and Unit Clerks. Also schedules coverage where needed for Unit Clerks. Initiate and assist in completion of Performance Improvement studies. Destroy medical records according to legal regulations. Maintain log on destroyed records. Maintain a list of all credentialed physicians and allied health professionals. This includes complete files on licensure/certification/registration on all the above and updating as necessary. Conduct bi-monthly Unit Clerk meetings, or more frequently as required. Maintain daily census and statistical reports as required. Complete monthly facility death list and mail to Metro Health Department. Any other tasks as requested by supervisor.

Job Requirements:
Job Qualifications Required: Accredited Records Technician or higher. High school graduate or equivalent. At least one year’s experience in a record department. Desired: A deep sincere respect for others. Good communication skills, written and verbal. Degree in Health Information management preferred
