The following must be completed and on file with the Program Director by the application deadline, please refer to the Information Packet for more details about the materials required for the application packet.

Official or unofficial copies of all post-secondary education transcripts.
Students with international transcripts must also submit a course-by-course transcript evaluation from an international evaluation transcript service that includes an overall GPA. Contact the Office of Admissions for assistance.

Submit at least two letters of recommendation in sealed envelopes using the reference letter form.

A resume or curriculum vitae.

Documentation of at least sixteen (16) observation hours in a general ultrasound department with a registered sonographer. All observation hours must be completed and documented using the clinical observation hours form and completed within 12 months of applying to the program.

Completion of the degree requirement
- Completion of all prerequisite or equivalent courses with a "C" or better:
- Anatomy and Physiology I
- Anatomy and Physiology II
- Introductory Physics
- College Algebra
- Medical Terminology

Completed application summary form.
Typically, a 3.0 GPA in all prerequisite courses is the minimum standard to be considered a competitive candidate.
